Before your first appointment you will be asked to complete a Client Information Form.  This form includes your contact information, as well as important questions about your health so that the therapist can adapt or modify their massage technique appropriately.  You will be asked to complete this form again whenever your contact or health information changes.  To stay current, we may ask that you update your Client Information Form every six months to stay current.
